By submitting this form, you agree to and accept the following terms and conditions:

The risk of injury to my child from the Soccer training involved is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

1. For myself, my family, and child, I assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my child’s participation; and, 
2. With respect to any and all injury, disability, death, or loss or damage to person or property incident to my
   child’s involvement or participation in this program, I will undertake full responsibility. TNSC, its members, all
   coaches, sponsors, and volunteers will not be liable for any injury, loss or damage of my child throughout all
   training sessions. In addition, it will be my full responsibility to drop off my child to the training ground,
   supervise him/her throughout the training session, and pick him/her up after the end of each training session.
3. I, the parent/guardian, declare that I have explained to my child: the risks of the activity, his/her responsibilities for adhering to the rules and regulations, and that my child understands this agreement.
4. By participating in this program, I will also give permission to TNSC and its members to release photographs, videos, and images of my child to be used in social media sites: Facebook, Twitter, etc.

I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
